By electing to participate in the ADT Water Leak Detection Program for State Farm Customers (referred to below as the Program), you confirm that you own, or otherwise have the authority to enter into a contract for ADT monitoring services at, the premises at which ADT monitoring services are provided and you have elected to participate in the Program to obtain Program discounts on the purchase and/or installation of ADT equipment and ADT monitoring services. You understand that (i) your participation in the Program is subject to the terms and conditions of your ADT Residential Services Contract and (ii) you are required to maintain an active homeowner’s insurance policy provided by State Farm for risk of covered loss at the premises for which ADT monitoring services are provided pursuant to your ADT Residential Services Contract. ADT’s extended limited warranty/quality service plan for Program participants is subject to additional charges. The Program discount for monthly service charges shown above will continue until the Program is canceled by ADT, at ADT’s sole discretion, upon 30 days prior notice to you after which time you will be given the option to self-monitor at no additional cost or purchase ADT monitoring services at standard rates.

You understand that, as part of the Program, information about you and your telematics and system usage will be used to help refine the products and services offered by State Farm and/or ADT. As a condition to your participation in the Program, you agree that ADT may share with State Farm any information about your account, available telematics, and any specific data regarding your system usage (excluding audio and video) to determine any discounts, rates, or other terms for products and services that they may offer, and for other internal business purposes, such as analyzing the impact of security systems and related products on customer characteristics and insurance claims, pricing and underwriting, and new product development. You agree that you may only revoke your agreement to this data sharing by contacting ADT at 800.238.2727, and that changing your general data sharing preference online will NOT revoke your agreement to share your data under the Program.

If (i) your State Farm homeowner’s insurance policy is not issued or is terminated for any reason or (ii) you revoke your consent for ADT to share your ADT system usage and other data described above, your participation in the Program will be terminated and you will be given the option to self-monitor at no additional cost or purchase ADT monitoring services at standard rates.
